Testimonial Release

PURITY PRODUCTS TESTIMONIAL RELEASE

By submitting an endorsement or testimonial to Water Pure, Inc. d/b/a PURITY PRODUCTS® (“Purity Products”) in any format, you grant your permission to Purity Products, and its parents, subsidiaries, representatives, assigns, affiliates, employees, and any person, corporation, or entity acting under its permission or authority, the worldwide, perpetual, transferrable, irrevocable right and permission to publish, reproduce, distribute, and otherwise use your name, image, voice, video, biographical information (including but not limited to your city and state of residence or title and place of business, if applicable), likeness and any opinion, statement or endorsement made by you (the “Testimonial”), in whole or in part, in any media now known or hereafter developed, including but not limited to the worldwide web, social media, television, and streaming services, at any time or times, for purposes of promoting, marketing, advertising, and sale of Purity Products’ products and services, in such manner and with such frequency as Purity Products shall determine, in perpetuity, without further compensation or consideration to, and without further authorization by, you.

You further acknowledge that the Testimonial shall constitute the property of Purity Products, who may alter, modify and adjust it, provided that any statements made by you are not thereby rendered inaccurate or misleading. You understand and agree that the Testimonial may be used in whole or in part and may be paraphrased, shortened, spoken by an actor, put in conversational form and dramatized, or be accompanied by other material.

You also affirm any statements or endorsement made or to be made by you in the Testimonial will be factually accurate and will represent your honest opinions, findings, beliefs, or experiences and do not violate the rights of any third party. You agree to promptly notify Purity Products in writing in the event your opinions underlying your Testimonial change such that your Testimonial would no longer be factually accurate.

You hereby waive all rights of inspection or approval with regard to the Testimonial, and to the extent permissible by applicable law, you hold harmless and release Purity Products and its representatives, assigns, affiliates, and employees from all claims, demands and causes of action which you, your heirs, representatives, executors, administrators or any other persons acting on your behalf have or may have by reason of this authorization.

You affirm that you are at least 18 years of age. You acknowledge and agree that this Testimonial Release constitutes a binding legal agreement between you and Purity Products, and you represent and warrant that you have full right, power, and authority to enter into this agreement. By submitting a Testimonial, you acknowledge that you have completely read, fully understand, and freely accept the terms of this Testimonial Release. This Testimonial Release may be executed and delivered by electronic means, which shall have the same force and effect as if it were an original signed instrument and the electronic copy shall be treated for all purposes as the original.
